Your First Visit
Upon arrival you will be asked to complete a patient history
form, which will include pertinent information concerning your health, both
medically and dentally. You can download these forms
order to make an efficient analysis of your dental needs, a clinical
examination and appropriate x-rays will be taken. Patients are encouraged to
bring previous records from their existing dentist. This will help give your
new dentist a timeline regarding your previous treatment and what dental
services you received. Also your existing x-rays may be useful and new x-rays
